1 Develop a Comprehensive Change Management Plan

Purpose: The objective is to ensure that project leadership is aware of and aligned with milestones in the Change Management Plan

Input: Communication Plan, Learning and Development Plan, Measurement and Benefit Realization Plan, Project schedule and plan, Stakeholder Engagement plan

Purpose: Monitoring performance and adheence to deliverables generated from the CM plan enables adjustments of the plan in response to performance

3 Review and Approve the Change Management Plan in Collaboration with Project Leadership

Purpose: Recommended that the relationship between the PM and the CM be defined early in the project, when the governance structure is established

Input: Resource Plan, Sponsorship Plan, Stakeholder Engagement Plan, Communication Plan, Impact Assessment and Readiness Plan, Learning and Development Plan, Measurement and Benefit Realization Plan, Sustainability Plan

Output: Change Management Plan Updated / Learning and Development Plan (updated) / Measurement and Benefit Realization Plan (updated) / Stakeholder Engagement Plan ( updated)

Output: Change Management Plan

Input: Change Management Plan, Project Plan

Purpose: Includes required actions and baseline measures as they pertain to a change effort's scope, expected benefits, role requirement, resources, activity schedule, risk, and measurement

Output: Change Management Plan (approved)

4 Develop feedback Mechanisms to Monitor Performance to Plan

Input: Change Management Plan, Project Charter and Project Documentation, Project Schedule and Plan
