Avis needed more than touching assistance from her nurse to bend her right knee as she used a bedrail to roll onto her left side, roll onto her right side, and return to her back. What would be her score for rolling left and right?

Avis rolled from her back to her side before pushing up with her arm to sit upright on the side of the bed. She required assistance lifting her R leg out of bed. What score would this indicate for supine to sit?

When getting back into bed, Avis requires assistance to lift both legs into the bed as well as assistance at her trunk. What would the score for sit to supine be?

After Avis' femur fracture, she is TDWB on the R LE. When coming from a seated position to a standing position, Avis required more than half of the assistance of the PT. What score would this indicate?

The PT puts a chair next to the bed, Avis stands and pivots into the chair with the PT doing less than half the effort while maintaining her TDWB. What score would this be for bed to chair transfers?

During the evaluation, Avis reports she needs to use the commode. The PT places a bedside commode next to Avis in the chair and she stands and pivots to the commode and requires contact guard assistance on the ascent and descent.

As Avis starts to ambulate, she requires the PT to provide physical assistance with every step to maintain TDWB as well as advancing her FWW. She ambulates 15 feet. What score would this be regarding walking 10 feet?

Avis and the PT ambulate out of her room, turn 90 degrees, walk down the hallway and then make another 90 degree turn into the gym. In total she ambulates 75' and needed assistance less than half the time but more than steadying assistance.

Once in the therapy gym, Avis and the PT walk the length of the hallway (150') with one brief standing rest break. Towards the last half of the walk, Avis needs more than half the help to advance the walker due to fatigue. What level is this?

When asked by the PT, Avis mentions that she does not have any steps at home and does not go into buildings that aren't accessible with ramps or elevator access. What would you score Avis for 1 Step, 4 Steps, and 12 Steps?

Avis also states to the PT that she does not use a wheelchair at home but with her WB status, she may use one here. She is able to propel the wheelchair without physical assistance 50' with turns but does need cues for safety.

While wheeling herself in the hallway, Avis tells the PT she cannot propel herself anymore and needs assistance. The PT propels Avis the remaining 50' of the 150' goal. What score would this be for wheeling 150'?
