Everyone should begin discussing the follow-up needs and activities during the preparation phase of the FGC process.

Regarding the FGC plan, the casework staff and service providers are less likely than the family to follow through with their agreed tasks within the plan when it is in the process of being implemented.

The worker partners with representatives from the family to talk about how the plan is going through continual calls, visits, and e-mails. As a result, the worker will likely be the first person outside the family to know if the plan is working.

As long as the family’s plan meets the statutory requirements, it is the agency’s obligation to resource the plan, in collaboration with other system providers.

One reason to have follow-up conferences is to celebrate successes.

A follow-up conference should be offered to the family whenever there are new decisions to be made.

If the family plan is not going well, the family should not be given the chance to have a follow-up conference to revise their plan.

The only way changes can be made to the family plan is if another conference is held.

If the worker doesn’t agree with the family plan (that meets all statutory requirements), they can choose not to implement the plan.

A follow-up conference can be requested at any time by anyone involved in the plan; the family group has the final say about whether one is needed.

Follow-up activities automatically stop 3 months after the initial FGC conference.

In order to determine if a follow-up conference is needed, the family needs to be kept current on the agency’s thinking and actions, in addition to progress and changes on the case, in a transparent manner by the worker.
