คู่สัญญาฝ่ายผู้กู้ (ฝ่าย ข.) ชื่อ Anucha NamSommuti เพศชาย อายุ 33 ปี ภูมิลำเนาอยู่เลขที่ 123 ตำบลช้างเผือก อำเภอเมือง จังหวัดเชียงใหม่ ประเทศไทย

คู่สัญญาทั้งสองฝ่ายได้ตกลงกันโดยสุจริตและสมัครใจ จึงได้ทำหนังสือสัญญาฉบับนี้ โดยมีข้อกำหนดดังต่อไปนี้

ฝ่าย ก. ตกลงให้กู้ยืมแก่ฝ่าย ข. เป็นเงินจำนวน 200,000 บาทถ้วน (สองแสนบาทถ้วน)

และต่อจากนั้นให้ฝ่าย ข. ชำระหนี้ในทุกวันที่ 1 ของเดือน จนกว่าจะชำระหนี้เงินกู้เสร็จสิ้นครบถ้วนทั้งหมด

หากฝ่าย ข. ผิดนัดไม่ชำระหนี้งวดใดงวดหนึ่งตามกำหนดเวลา ฝ่าย ก. มีสิทธิเรียกร้องให้ฝ่าย ข. ชำระหนี้เงินกู้ที่เหลืออยู่ทั้งหมดโดยทันที

หากฝ่าย ข. ชำระหนี้เกินกำหนด ฝ่าย ก. มีสิทธิเรียกร้องให้ฝ่าย ข. ชำระดอกเบี้ยสำหรับหนี้เงินกู้ที่ค้างชำระ โดยคำนวณตามอัตราร้อยละหกต่อปี

หนังสือสัญญาฉบับนี้ได้จัดทำขึ้นจำนวนสองฉบับ คู่สัญญาทั้งสองฝ่ายถือไว้ฝ่ายละหนึ่งฉบับ และให้มีผลบังคับทางกฎหมายเสมอกันทุกประการ

คู่สัญญาทั้งสองฝ่ายได้อ่านและเข้าใจข้อความในหนังสือสัญญาฉบับนี้โดยตลอดแล้ว จึงได้ลงลายมือชื่อไว้เป็นหลักฐาน
