Feet and Legs
Inspect and palpate hips, thighs, calves, and feet
Assess tone, temperature, color, hair distribution, and edema
Check capillary refill on one toe of each foot
Palpate femoral, popliteal, posterior tibial, and dorsalis pedis pulses
Assess ROM of hips, knees, ankles, and feet
Assess strength of thighs, legs, and feet against resistance
Spine
Ask patient to bend forward as far as comfortable
Palpate spinal processes under clothing
Place hands on scapulae and hips to assess scoliosis
Assess standing posture for kyphosis and lordosis
Chest and Lungs
Inspect chest skin and expansion
Inspect breathing effort, depth, rhythm, and pulsations
Palpate chest wall
Auscultate anterior breath sounds systematically
Inspect posterior thorax and spine skin
Palpate posterior thorax
Auscultate posterior breath sounds systematically
Heart
Recall valve sites: Aortic, Pulmonic, Erb’s, Tricuspid, Mitral
Auscultate aortic area
Auscultate pulmonic area
Auscultate Erb’s point
Auscultate tricuspid area
Auscultate mitral area / apical pulse
Abdomen
Ask about last BM, constipation, diarrhea, blood, or habit changes
Inspect abdominal skin and contour
Auscultate bowel sounds in all four quadrants
If absent, listen 5 minutes in each quadrant
Auscultate aorta, renal, and iliac arteries for bruits
Percuss all four quadrants for tone
Lightly palpate all four quadrants
Deeply palpate all four quadrants
Identify organs in correct abdominal quadrants
Assess urinary color, clarity, and urinary symptoms
For women, assess menstrual regularity, pain, bleeding, and changes
