At Corona , Andrés had recently joined the production team as a quality control assistant . The company was working on a long - partnership with a construction firm that ordered ceramic tiles for luxury apartment projects across Colombia . Because of an upcoming deadline , the production department was practically running on after several weeks of overtime .

One afternoon , Andrés up with a senior technician to inspect a large batch of tiles before shipment . Their goal was to up positive feedback from the client and the business relationship . However , while rushing through the inspection process , Andrés overlooked a calibration problem in one of the measuring machines . Confident that everything was under control , he approved the shipment without requesting an additional review .

A few days later , the client complained that some of the tiles were uneven and difficult to install . The issue caused delays at the construction site and created bad between the client and the company . During an emergency meeting , Andrés decided to face the and admitted that he had skipped part of the quality control procedure .

At first , Andrés was worried that he might fall with his supervisor and coworkers because of the mistake . However , instead of blaming him completely , the team focused on finding ways to turn the situation . With hindsight , Andrés realized that the pressure to work faster had clouded his judgment . He also understood that the risks of skipping safety checks always the benefits of saving a little time .

To prevent similar problems in the future , Andrés proposed a double - check system where two employees would review machine settings together before production started . Through this and error experience , he learned that small mistakes can have serious consequences in manufacturing , especially when quality and reputation are involved .
