Before DNA replication, a chromosome is one long molecule of DNA.

After replication (in Interphase), that single strand copies itself. It becomes a chromosome made of two identical sister chromatids stuck together at a centromere.

During Prophase I of meiosis, the maternal double-stranded chromosome and the paternal double-stranded chromosome find each other and squeeze together. This bundle of 4 chromatids (2 sisters + 2 sisters) is called a tetrad.

When the chromosomes are packed into that tetrad, the inner non-sister chromatids physically overlap, break, and swap pieces of DNA.

When the homologous pairs line up in the middle of the cell, they do so completely randomly. Mom's chromosomes don't all stay on the left, and Dad's don't all stay on the right. They mix randomly, meaning every gamete gets a unique combination of you

is a physical error during cell division. Instead of chromosomes separating smoothly to opposite sides of the cell, a pair fails to separate.

The Result: Gametes with the wrong number of chromosomes. If a gamete with an extra chromosome (n+1) meets a normal gamete (n), the resulting zygote will have 3 copies of that chromosome instead of 2.
