Explicación

La presentación clínica (secreción acuosa, folículos, adenopatía preauricular, brote en comunidad) es típica de conjuntivitis viral por adenovirus, responsable del 65-90% de los casos virales. HSV-1 es menos frecuente (1.3-4.8%) y suele ser unilateral con lesiones vesiculares.

La tinción de Gram es útil en sospecha bacteriana (secreción purulenta), no en viral. En este caso, el diagnóstico es clínico y las pruebas virales pueden confirmar pero no cambian el manejo inmediato salvo en casos graves o atípicos.

La conjuntivitis viral por adenovirus es autolimitada. El tratamiento es sintomático (compresas frías, lágrimas artificiales) y medidas higiénicas para reducir contagio. Los antibióticos no están indicados. Los esteroides están contraindicados en fase aguda sin evaluación oftalmológica.

La conjuntivitis viral es extremadamente contagiosa, mucho más que la bacteriana (la cual suele dar brotes más limitados).
