An "alert and oriented" patient knows his or her name, where he or she is, and the correct date.

Starts with V

A patient is "responsive to verbal stimuli" even if he or she reacts only to loud sounds.

Starts with P

Response to pain is tested by pinching the patients ear lobe or skin over the collarbone. A patient who withdraws from the stimulus is said to be "responsive to painful stimuli"

Starts with U

The patient does not respond to either verbal or painful stimuli.
