Feedback

Indirect hernias enter via the deep inguinal ring, then pass along the canal toward the scrotum/labia. See Table 7.1 and related notes.

The superficial inguinal ring is guarded posteriorly by the conjoint tendon, while the deep ring is guarded by internal oblique fibers.

Direct hernias occur through the posterior wall within Hesselbach’s triangle (medial to inferior epigastric vessels).

Direct hernias typically have a wider neck; indirect hernias have a narrower neck as they pass through the deep ring.

In males, the spermatic cord traverses the canal; in females the round ligament does so instead.

The base of the superficial inguinal ring is bounded by the pubic crest; apex directed upward and laterally.

Spermatic cord contents include ductus deferens, testicular/cremasteric arteries, pampiniform plexus, nerves, and lymphatics.

The lacunar ligament forms the medial boundary/floor of the medial part of the inguinal canal.

Increased intra-abdominal pressure closes the canal like a flap via the flap valve mechanism.

The deep inguinal ring is the inlet to the canal and is an opening in fascia transversalis, located above the midinguinal point.
